Changes in Relevant for UDS 2026

We are updating Relevant to support the 2026 UDS reporting year. We release changes incrementally as they are completed and update this page to reflect what is available.
Timeline of updates
- December 2025. HRSA released the proposed UDS changes for calendar year 2026.
- April 20, 2026. We enabled the UDS 2026 measures for all health centers.
- April–May 2026. We refreshed the CQM and RxNorm value sets.
- Spring–August 2026. We added initial updates to the UDS module based on the proposed changes.
- September 2026. HRSA released the 2026 UDS Manual. We began reviewing the final reporting instructions and identifying any additional changes needed in Relevant.
- September–October 2026. We are incrementally releasing the remaining updates and expect to complete the planned UDS module changes by the end of October. We will notify health centers when this work is complete.
Updates to the UDS module and updates to data mappings may happen at different times. A new or revised line appearing in the module does not necessarily mean that its default mapping is available or that a health center’s mapping has been reviewed and validated.
Table 5: Selected Service Detail Addendum
Oct 2026. Both Mental Health Service Detail and Substance Use Disorder Detail sections have been removed to comply with the manual, which states:
The Selected Services Detail Addendum is being removed to reduce reporting burden on health centers.

Line Revisions
May 2026. Line 26, Health supervision of infant or child (ages 0 through 11), was renamed to Well Child Visits (ages 0 through 11). We also updated Relevant to count this line using the Well Child Interventions Data Element instead of Visit Billing or Diagnosis Codes for patients under age 12 at the start of the intervention. The data element itself does not require a linked visit, but the patient must still qualify for UDS reporting in the reporting year to be included in Table 6A.

Table 6B: Quality of Care Measures
New Section
We added Section O, Screening for Future Falls Risk, to Table 6B per the 2026 UDS Manual. Line 24 reports the number of patients aged 65 and older, records reviewed, and patients screened for future fall risk.
Tobacco Use Screening and Cessation Intervention
September 2026.
- Previously, a hospice intervention with no end date was not treated as ongoing when the measure checked whether it overlapped the measurement period. The measure now treats a missing end date as ongoing through the end of the measurement period.
- Tobacco use screens recorded more than once for the same patient on the same date are now deduplicated, with a positive screen taking precedence over a negative screen. The
TobaccoUseScreendata element’s library default mapping has also been updated to treat ICD-10 codesF17.201,F17.211,F17.221, andF17.291as negative tobacco-use screens because they indicate tobacco use in remission. These updates apply to the 2025 and 2026 Tobacco Use Screening and Cessation Intervention measures and their related care gaps.
Line Revisions
June 2026. We updated Table 6B Section M to replace the old line 22, Dental Sealants for Children between 6-9 Years, with the two 2026 measures from the Program Assistance Letter:
