Risk Models Overview

In Relevant, Risk Models are used to translate patient data into risk scores and risk levels that support care planning, prioritization, and population management.

Health centers commonly use risk models to:

  • Identify patients who may benefit from care management services or more intensive care support
  • Inform visit planning decisions, such as visit frequency or follow-up needs
  • Adjust provider panels to better accommodate high-risk patients

There are many validated approaches to risk stratification, each with strengths and weaknesses. Relevant supports both standardized (for example, the 10-Year ASCVD Risk Model and the Charlson Comorbidity Index (CCI)) and custom Risk Models, recognizing that health centers serve patients and communities with unique needs.

Note: Support for the CCI Risk Model is available as an add-on to Relevant through an agreement with Cornell University.
