About Relevant

Relevant is a web-based data analytics and population health platform designed for community health centers. Relevant provides and hosts a centralized data warehouse that extracts, normalizes, and integrates data from multiple source systems, including EHRs and other clinical, operational, and reporting data sources.

The platform transforms this data into standardized and configurable datasets that power dashboards, reports, and analytical workflows used for regulatory reporting, quality improvement, population health management, and clinical and operational decision support. Relevant supports both out-of-the-box content and customization to meet the specific reporting and operational needs of each organization.

Relevant is designed for use across roles within a health center, including providers, care teams, front desk staff, analysts, and administrators. Access controls and role-based permissions ensure users can securely interact with data appropriate to their responsibilities.
